تاثیر تمرینات ورزشی ترکیبی با دو الگوی متفاوت بر سطوح پلاسمایی اینترلوکین 15، نیمرخ لیپیدی و شدت استئاتوز در زنان چاق یا دارای اضافه‌وزن مبتلا به کبد چرب

چکیده

زمینه و هدف: اینترلوکین 15 (IL-15) سایتوکاینی است که به عنوان یک شاخص آنابولیک عضلانی شناخته می­شود؛ با این وجود، التهاب ناشی از IL-15، بیماری کبد چرب غیرالکلی را  تشدید می­کند. امروزه استفاده از درمان­های کمکی غیر دارویی مورد توجه متخصصان سلامت می­باشد. هدف از این پژوهش، بررسی تاثیر تمرینات ترکیبی با دو الگوی متفاوت بر سطوح پلاسمایی IL-15، شدت استئاتوز کبدی و نیمرخ لیپیدی در زنان چاق یا دارای اضافه وزن مبتلا به کبد چرب بود.
مواد و روش‌ها: تعداد 33 زن مبتلا به کبد چرب (سن 5/7 ± 43 سال و شاخص توده بدنی 6 ± 84/32 کیلوگرم بر متر مربع) به صورت تصادفی به دو گروه تمرینی و کنترل تقسیم شدند. گروه تمرین ترکیبی1، تمرین مقاومتی دایره ای همراه با تمرین اینتروال هوازی (5-2 ست 4 دقیقه­ای، شدت 75-70 درصد حداکثر ضربان قلب) و گروه ترکیبی 2، تمرین مقاومتی دایره ای به همراه تمرین اینتروال شدید (13-8 ست یک دقیقه­ای، 95-85 درصد حداکثر ضربان قلب) را به مدت 12 هفته، اجرا کردند. سنجش متغیرهای آنتروپومتریک، اولتراسونوگرافی و نمونه­گیری خونی قبل و بعد از مداخله انجام شد.
نتایج: هر دو نوع تمرین ترکیبی باعث کاهش معنادار درصد چربی بدن شد (001/0>P). تمرین ترکیبی 1 باعث کاهش معنادار (008/0=P) و تمرین ترکیبی 2 باعث کاهش غیرمعنادار شدت استئاتوز کبدی نسبت به گروه کنترل شد (07/0=P). بین دو گروه تمرین ترکیبی و گروه کنترل در میزان IL-15 تفاوت معناداری وجود نداشت. در سایر شاخص­های آنتروپومتریکی و نیمرخ لیپیدی تفاوت معناداری بین گروه­ها مشاهده نشد.
نتیجه‌گیری: با توجه به نتایج، هر دو نوع تمرین ترکیبی، تغییر معناداری در مقادیر IL-15 پلاسما و نیمرخ لیپیدی ایجاد نکرد، اما سبب بهبود در شدت استئاتوز و درصد چربی بدن در افراد چاق یا دارای اضافه­وزن مستقل از کاهش وزن شد. بنابراین به نظر می­رسد ترکیبی از تمرینات مقاومتی و اینتروال هوازی در تحقیق حاضر، احتمالا بتواند به عنوان یک درمان غیردارویی کمکی در کاهش شدت استئاتوز کبدی و بهبود بیماران کبد چرب مورد استفاده قرار گیرد.

Background & Purpose: Interleukin 15 (IL-15) is a cytokine that is known as an anabolic factor for skeletal muscle. However, Interleukin-15-mediated inflammation promotes non-alcoholic fatty liver disease. Today, the use of non-pharmacological adjuvant therapies is considered by health professionals. The purpose of this study was to investigate the effect of combined training with two different patterns on plasma IL-15 levels, severity of hepatic steatosis and lipid profile in obese or overweight women with fatty liver.
Methodology: 33 women with fatty liver (age 43.45 ± 7.57 and Body Mass Index 32.84 ± 6) were randomly divided to training groups and control. Combined training group 1 performed circular resistance training with aerobic interval training (2-5 intervals of 4 minutes, intensity of 70-75% of maximum heart rate) and combined training 2 performed circular resistance training with high intensity interval training (8-13 intervals of 1 minutes, 85-95% maximum heart rate) for 12 weeks. Measurement of anthropometric variables, ultrasonography and blood sampling were performed before and after the intervention.
Results: Both types of combined trainings significantly reduced body fat percentage (P<0.001). Combined training 1 caused a significant reduction (P=0.008) and combined training 2 caused a non-significant reduction in the severity of hepatic steatosis compared to the control group (P=0.07). There was no significant difference in IL-15 levels between the combined training groups and the control group (P<0.05). There was no significant difference between the groups in other anthropometric indices and lipid profile.
Conclusion: According to the results, it seems that a combination of resistance and aerobic interval training in the present study, can probably be used as an adjuvant non-pharmacological treatment to reduce the severity of hepatic steatosis and improve fatty liver patients.
