تأثیر تمرینات هوازی و مقاومتی بر برخی سایتوکین‌ها در بیماران مبتلا به کبد چرب غیر الکلی، مرور سیستماتیک و فراتحلیل

چکیده

اهداف: بیماری کبد چرب غیر الکلی (NAFLD) شایع­ ترین نوع بیماری کبدی در جهان است. هدف از این پژوهش تعیین و بررسی تأثیر تمرینات ورزشی هوازی و مقاومتی بر برخی از سایتوکاینها در بیماران مبتلا به کبد چرب غیر الکلی بود.
روش مطالعه: این پژوهش یک مطالعه مروری نظام‌مند و فراتحلیل بود. بعد از غربالگری اولیه، متن کامل مقالات ارزیابی و مقالات دارای معیار ورود به پژوهش آنالیز شدند. از تعداد ۱۲۰ مقاله مورد بررسی 12 مقاله معیار ورود به مرور سیستماتیک و فراتحلیل را کسب کردند. میانگین سنی آزمودنی‌ها در پژوهش حاضر ۱۰± 52 سال بود. اثرات مداخله با استفاده از تفاوت میانگین استاندارد شده (SMD)  و مدل اثرات تصادفی ارزیابی گردید. داده‌ها با استفاده از آزمون‌های تعیین اندازه اثر، تحلیل سو‌گیری و ناهمگنی و با استفاده از نرم‌افزار  STATA  انجام شد.
یافته ­ها: نتایج فراتحلیل تنها تاثیر معنی­دار سطوح CRP بر اثر تمرین استقامتی با شدت متوسط و بالا و تمرین مقاومتی با شدت متوسط را نشان داد. همچنین، نتایج فراتحلیل نشان داد تمرین استقامتی با شدت متوسط و بالا و تمرین مقاومتی با شدت متوسط در کل باعث بهبود معنی­دار سطوح اینترلوکین۶ ، اینترلوکین۸ ، آدیپونکتین و TNF-α  نمی­گردد.
نتیجه گیری: نتایج تنها نشان دهنده تاثیرتمرین استقامتی با شدت متوسط و بالا و تمرین مقاومتی با شدت متوسط بر سطوح شاخص CRP به عنوان حساس­ترین شاخص التهابی بود. باتوجه به نتایج این فراتحلیل و با توجه به محدودیت تعداد مطالعات در این زمینه نیاز به تحقیقات بیشتر و با کیفیت بالاتر احساس می‌شود.

چکیده [English]

Objectives: Non-alcoholic fatty liver disease (NAFLD) is the most common type of liver disease in the world. The purpose of this study was to investigate the effect of aerobic and resistance training on some cytokines in patients with non-alcoholic fatty liver disease.
Methods: This research was a systematic review and meta-analysis. After the initial screening, the full text of the evaluation articles and the articles with criteria for entering the research were analyzed. Out of the 120 reviewed articles, 12 articles met the criteria for inclusion in the systematic review and meta-analysis. The average age of the subjects in the present study was 52 ± 10 years. The effects of the intervention were evaluated using the standardized mean difference (SMD) and the random effects model. Data were analyzed using effect size tests, bias and heterogeneity analysis and using STATA software.
Results: The results of the meta-analysis showed only a significant effect of CRP levels as a result of moderate and high intensity endurance training and moderate intensity resistance training. Also, meta-analysis results showed that moderate and high intensity endurance training and moderate intensity resistance training in general do not significantly improve interleukin-6, interleukin-8, adiponectin and TNF-α levels.
Conclusion: The results only showed the effect of moderate and high intensity endurance training and moderate intensity resistance training on CRP index levels as the most sensitive inflammatory index. According to the results of this meta-analysis and considering the limited number of studies in this field, the need for more and higher quality research is felt.
