بررسی تاثیر فعالیت هوازی بر سایتوکین ضد التهابی اینترلوکین-10(IL-10)، رادیکال آزاد نیتریک اکساید (NO)، خستگی و کیفیت زندگی در زنان مبتلا به مولتیپل اسکلروزیس

چکیده

سابقه و هدف: مولتیپل‌اسکلروزیس (MS) شایع‌ترین بیماری سیستم عصبی مرکزی است. این بیماری اثرات منفی بر سلامت جسمی و روانی دارد. سایتوکین ها و رادیکال‌های آزاد نقش مهمی در پاتوژنز این بیماری دارند و مهمترین هدف مداخلات درمانی می‌باشند. فعالیت ورزشی منظم افت عملکردی مرتبط با بیماری MS را کاهش می‌دهد. اما تاثیر ورزش بر سایتوکین ها و رادیکال های آزاد مرتبط با فعالیت در این بیماری به طور کامل مشخص نشده است. هدف از این مطالعه بررسی تاثیر 8 هفته فعالیت ورزشی هوازی بر سطح IL-10،NO ،خستگی و کیفیت زندگی در زنان مبتلا به MS بود.
مواد و روش‌ها:30 زن مبتلا به MS با میانگین سن 6.97 ± 30.11 سال و شدت بیماری(EDSS) کمتر از 3 به طور داوطلبانه در این مطالعه شرکت کردند.آزمودنی‌ها بصورت تصادفی به دو گروه ورزش (n=15) و کنترل (n=15) تقسیم شدند. گروه تمرین برنامه ورزش هوازی را 3 روز در هفته و به مدت 8 هفته اجرا کردند. نمونه‌گیری‌های خونی در سه مرحله(هفته 0، پایان هفته چهارم و پایان هفته هشتم) و تکمیل پرسشنامه‌ها در دو مرحله قبل و بلافاصله  بعد از اتمام آخرین جلسه تمرین انجام شد. داده‌ها با استفاده از آزمون‌های آماری تحلیل کوواریانس با اندازه‌گیری‌های تکراری، تحلیل کوواریانس و تی مستقل در سطح معنا داری p≤0.05 تجزیه و تحلیل شدند.
 یافته ها: نتایج نشان داد که پس از 8 هفته فعالیت هوازی NO در گروه تمرین کاهش معنی‌دار یافت (p≤0.05). IL-10 گروه تمرین پس از 8 هفته در مقایسه با گروه کنترل افزایش معنی دار داشت (p≤0.05). خستگی در گروه تمرین نسبت به گروه کنترل پس از 8 هفته فعالیت کاهش معنی دار و کیفیت زندگی افزایش معنی دار داشت (p≤0.05).
نتیجه‌گیری: فعالیت ورزشی هوازی با شدت کم تا متوسط (40 تا 70 درصد حداکثر اکسیژن مصرفی) و متناسب با توانایی بیماران MS به‌گونه‌ای که باعث تشدید علائم بیماری از جمله حساسیت گرمایی و خستگی نشود، می‌تواند از طریق بهبود خستگی،کیفیت زندگی و افزایش سایتوکین‌های ضدالتهابی مانند IL-10 و همچنین کاهش رادیکال‌های آزاد از جمله NO برای بیماران MS مفید باشد.

چکیده [English]

Background & purpose: Multiple Sclerosis (MS) is the most prevalent disease of the central nerve system. This disease has negative effects on physical and mental health. Cytokines and Free Radicals play an important role in the pathogenesis of Multiple Sclerosis and they are the major targets of therapeutic interventions. Regular exercise reduces functional loss associated with multiple sclerosis. However, the impact of exercise on cytokines and free radicals associated with disease activity is remained relatively unexplored. The aim of this study was to examine the effects of 8 weeks Aerobic Exercise Training (AET) on IL-10 and NO levels, fatigue and quality of life in female patients with MS.
Methodology: participants of the study were included 30 females with MS disease with average age of 30.11±6.97 and EDSS less than 3. They were randomly divided into control (n=15) and training (n=15) groups. The training group performed AET program, 3 days a week for 8 weeks. Results were analyzed by covariance analysis test through continual measures (ANCOVA), covariance and independent T-test at p≤0.05 significant level.
Results: The results showed that, IL-10 increased significantly in the exercise group and NO decreased significantly in the exercise group compare to control group. Also, Fatigue decreased significantly in exercise group compared to control group, following eight weeks of AET,. and quality of life increased significantly in exercise group versus control group following eight weeks of AET .
Conclusion: The results suggested that aerobic exercise training with low to moderate intensity (i.e., %40 to %70 of VO2max), according MS patientʼs ability so that, It does not exacerbate the symptoms of the disease, including heat sensitivity and fatigue, can be beneficial for MS patients through improving fatigue and quality of life and increasing in Anti Inflammatory cytokines like IL-10 and reduction in free radicals like NO.
